New fully insured product grid for groups 1 to 50, effective 4/1/17. The new Primary Advantage Plans have been added in package 21.

Primary Advantage Plans include:

• $0 copays for Virtual Visits
• $0 copays for Primary Care Physicians – All ages, not just children under 19
• $100 copays for Specialists
• $50 copays for Urgent Care
