New Forms Required

UnitedHealthcare has created new enrollment forms for UnitedHealthcare Level Funded plans.  The new enrollment forms will be required for all submitted business beginning with 11/1 effective dates. 

As a courtesy, we have compiled the forms and created separate enrollment packages for Small and Large group.
